While pain is the energy that moves us toward change, the absence of pain isn't enough to sustain it. This is one of the factors that makes change so hard: we experience the lack of pain as relief, as a kind of balance or homeostasis. We tend to rest there; we're comfortable again. We take a few pills, and our pain decreases. We see a therapist once or twice, and we don't go back. Many of us aren't really that interested in change. We just want a rescue from pain. Pleasure is what I think is on the other side of relief. In order to move us from pain to relief to change , human beings need regular, positive reinforcement. We need to really feel that our effort is giving us something new and different than just relief; it's creating a welcome, desired difference. And that difference needs to be sustained in order for us to trust our effort is working. We need positive, consistent reinforcement of our efforts. The Recession and Mental Health Care

The recession has kept people who need care out of therapy. That was the consensus of a small group of private practice therapists I met with Monday. We are a interesting and somewhat diverse group of folks: three licensed psychologists, three LMFTs and a social worker. Each of us has an individual private practice in the southwest Twin Cities, and we meet every month or so in each others' offices for support, case consultation and resource sharing. As our meeting was coming to a close, one of us looked around and said, "I think people are coming to therapy less often, and when they do come, they are worse." All of us agreed. We are seeing that in our new clients: more severe symptoms of anxiety and depression; people who need medication taking themselves off and showing up in therapy; couples who might have been helped out of their patterns two years ago, making appointments like a "Hail Mary" touchdown attempt and often not even showing up to their first s...

More Deadlines

It's Thursday night, and I have more writing deadlines. I'm late on my professional blog post at GoodTherapy.org (again), and I have a sermon to do for Sunday. For the first 12 years of my parish ministry life, I preached about 46 Sunday sermons a year. I had time away for vacation, and occasionally for continuing education, or a bout with laryngitis, or a special guest preacher, but otherwise, I had a deadline every week. In addition, each year I had a half dozen Lenten sermons to write, a dozen or more funeral sermons to prepare, half a dozen wedding sermons, and a dozen or more newsletter columns to do. When I joined a staff for my last 8 years in the parish, the rhythm slowed to about every third Sunday plus the added services which I led. That's a lot of writing to the clock. That's a lot of writing, period . I know I learned to cope with this demand while an English major in college. What I remember most about my Major are the piles of papers I had to produce in...
